In accordance with clause 3.3 of the Regulations on Membership in the Association of Exporters of Medical Equipment of Domestic Production, approved by By the General Meeting of Members (Protocol No. 3 dd. 03.03.2017), a legal entity wishing to become a member of the Association submits the following documents to the Association at its location:
- application from the head of a legal entity or a person authorized to act on behalf of a legal entity on the basis of a power of attorney for membership in the Association in the form approved in Appendix No. 1 to the Regulations (on letterhead with the signature and seal of the authorized person) (original);
- The Charter of a legal entity (a copy certified in accordance with the procedure established by law);
- certificate of state registration of a legal entity (a copy certified in accordance with the procedure established by law);
- certificate of registration of a legal entity with a tax authority (a copy certified in accordance with the procedure established by law);
- a document confirming the authority of the head. If another person acts on behalf of a legal entity, powers of attorney issued to an individual or individuals for performing actions on behalf of the legal entity are also submitted. If the specified power of attorney is signed by a person authorized by the head, a copy of the document confirming the authority of this person (a copy certified in accordance with the procedure established by law) must be submitted;
- the decision (protocol) of the sole participant (meeting of participants, shareholders) of the legal entity approving the entry of the legal entity into the membership of the Association (original);
- at least one (1) export contract (or extracts from it) for the supply of medical equipment of domestic production (copies certified in accordance with the procedure established by law).
A candidate for membership of the Association is obliged to pay an entrance fee within 30 (thirty) days from the date of the decision of the General Meeting of Members of the Association on admission to membership of the Association. The annual membership fee is paid by each candidate for membership of the Association at the same time as the entrance fee. The entrance fee is 10,000 (Ten thousand) rubles 00 kopecks. The annual membership fee is 10,000 (Ten thousand) rubles 00 kopecks.
